scenraio test case, which often be used to simulate user's real enviroment, sometimes comes from defect report by end user, or sum up from daily adhoc testing.
In most of the time, scenraio test case are too complex to include in the normal functionality test case.
Functionality test case, in my humble opinion, comes from high level specification of software, detail steps to verify software if meet spec.
